FOR BUSINESSES
Important note to New Clients:
After completing this form, you will receive a confirmation of your submission. In this confirmation email you will be requested to respond with the following documents attached:
An original or certified copy of your driver’s licence or birth certificate. This means a copy certified by a JP or an original we can sight at our office.
Notice from ATO, Centrelink or other government agencies that contains your name and residential address issued within the last 12 months; or Council rates notice or utilities bill within the last 3 months showing the individual’s full name and residential address; or Medicare card; or Electoral roll details.
Identify the individuals as per above guidelines.
For the entity:
Its full name
Its ABN;
Its ACN (to be verified by ASIC company extract); and
Authority allowing the individual to act on its behalf (e.g. company extract showing the individual as an office holder, trust deed, partnership agreement, employment contract, or confirmation of the person’s position from the entity’s website).
