Important note to New Clients:

After completing this form, you will receive a confirmation of your submission. In this confirmation email you will be requested to respond with the following documents attached:

  • An original or certified copy of your driver’s licence or birth certificate. This means a copy certified by a JP or an original we can sight at our office.

  • Notice from ATO, Centrelink or other government agencies that contains your name and residential address issued within the last 12 months; or Council rates notice or utilities bill within the last 3 months showing the individual’s full name and residential address; or Medicare card; or Electoral roll details.

Your application cannot be accepted until we receive these documents.If you have any questions or require further information, please do not hesitate to contact us.
